How to Claim Prizes
Cash 5 Winners of $599 or Less
Check with your local retailer to see if they’ll award you your prize, or visit and claim at any of our four Lottery Claim Centers, or mail your signed winning ticket to Colorado Lottery, PO BOX 7, Pueblo, CO 81002. Please allow one week for processing, and a check will be mailed to you.
Cash 5 Winners of $599 or More
You must claim at a Colorado Lottery Claim Center. Complete the Colorado Lottery Prize Claim Form and bring a photo I.D. with you. You’ll be asked to provide your social security number. Or you can opt to mail your signed winning ticket to Colorado Lottery, PO BOX 7, Pueblo, CO 81002 along with your Colorado Lottery Prize Claim Form. Fill out the form completely and sign it. Allow one week for processing, and a check will be mailed to you. Incomplete forms may result in a delay.
Cash 5 Jackpot Winners
Cash 5 jackpot winners must claim their prize at any Colorado Lottery Claim Center.
Splitting your jackpot prize?If you decide to “split” the prize, one person is designated as the prize recipient, signs the winning Cash 5 ticket, and distributes the money. All prize winners complete IRS Tax Form 5754 that will dictate how to split certain gambling winnings. If you decide to “split” the prize, you’ll need to provide the names, addresses, social security numbers, and the gross amount to be distributed to each person.
Come prepared.Complete the Colorado Lottery Prize Claim Form. Bring a photo I.D. and your Social Security number, if you have one. Acceptable identification includes: driver’s license, passport, military ID card, green card – or any form of government-issued identification with photo.
What you can expect when you win the jackpot.As part of the Open Records Act, we are required to release to the public your name, hometown, amount you won and the game you played. This information will be posted on coloradolottery.com and will be furnished to media upon request. You’ll be asked to have your picture taken which will appear on our “Wall of Winners’, our website, and our Facebook and Twitter social media outlets.

Complete a Colorado Lottery Claims Form before claiming prizes of $599 or more.
Deductions to Prizes $5,001+
29 percent will be withheld from your prize for taxes (25% federal; 4% state) if you provide us with your social security number.
34 percent will be withheld from your prize for taxes (30% federal; 4% state) if you do not provide us with a social security number.
If you have outstanding payments due to the Department of Human Services or Department of Justice, these will be deducted.
